BOB体育 Physicians earns Patient-Centered Medical Home recognition
The National Committee for Quality Assurance has recognized 11 University of Florida Health Physicians primary care sites as patient-centered medical homes, a model of care believed to improve the patient鈥檚 experience by emphasizing communication and coordination of care.
The recognition is awarded to primary care sites committed to following national standards that promote teamwork and the coordination of patient care and focus on building relationships between providers and patients. Eleven of BOB体育鈥檚 12 primary care sites were recognized with this designation in June 2015. The twelfth primary care site, BOB体育 Family Medicine 鈥� Magnolia Parke, has been recognized by the NCQA since October 2013.
"This recognition distinguishes BOB体育 practices among the leaders in advancing patient-centered care," said , senior associate dean, chief executive officer and chief medical officer of University of Florida Physicians, the clinical practice arm of the .
To become recognized by the NCQA as a patient-centered medical home, practices must follow six standards, which include patient-centered access, team-based care, population health management, care management and support, care coordination and care transitions, performance measurement and quality improvement. Each standard includes multiple measurements to assess whether the institution meets these requirements at every primary care location.
"Recognition by the NCQA as a patient-centered medical home is another step we are taking in order to ensure our patients are receiving the highest-quality medical care possible," said , chairman and professor of . "This recognition demonstrates that our health care professionals are committed to providing patient-centered care in all our clinics."
BOB体育 has 12 primary care locations across north central Florida offering comprehensive medical care for children and adults. The eleven primary care sites that received patient-centered medical home designation in 2015 will be up for re-accreditation from NCQA in June 2018.
