Before making an your appointment, you will need to complete the following:

• After watching the video, . Once you have successfully passed the quiz, you will be prompted to print a certificate. Fax the certificate to (352) 627-4173 before scheduling an appointment.
• Schedule your appointment
• Complete a New Patient Packet. You'll receive your packet by email. You will need to complete it and bring it with you to your appointment.
• The seminar may be attended in person. Please call (352) 265-0535 to register. In-person seminars are held at BOBÌåÓý Shands Cancer Hospital, 1515 SW Archer Road, Room 1204.
